Enroll with SWOP

SWOP produces some of the finest organically grown medical cannabis
in all of New Mexico and distributes it to qualified patients,
licensed in the New Mexico Medical Cannabis Program.

Enrollment into the S.W.O.P. New Mexico Medical Cannabis Program is painless, and pretty straightforward.

REQUIRED MATERIALS:
1. A valid New Mexico Medical Cannabis Program Patient ID Card.

2. A current and valid NM State recognized photo ID. (drivers license)

3. A completed, signed and verified S.W.O.P. enrollment form.

WHAT TO DO NEXT:
The quickest way to get enrolled with SWOP is to apply on-line. Download the SWOP enrollment form and fill out
the required information, sign it and email the form and copies of your cards to us at:

Emailed applications will receive an automated response with further information and a link to our
NEW CLIENTS WELCOME PAGE. There you can view actual SWOP medication, download newsletters and
catch up on all things SWOP. Only enrolled clients have acces to this page - let's keep it that way.

Good old-fashioned snail mail still works - it just takes a little longer.
Not that good with computers? No worries! Just send us the forms and photocopies in the US mail at:

Once the mandatory materials are sucessfully processed
your first purchase of medical cannabis may be arranged.
